I am in good physical condition and am able to participate in physical activities. I understand and freely and knowingly assume the risks inherent in the participation of physical activities, and as with all physical activities, recognize that there is an element of risk involved.
I hereby waive my right, claim or cause of action against the Somerset County Library System/ Watchung branch and release them from any liability for any injury, cost, damage, expense or claim which I or anyone on my behalf might have as a direct or indirect result of my participation in the program.
