Written in everyday language, the Five Wishes is America's most popular Advance Directive. This alternative form is acceptable as an Advance Directive in New Jersey. This Five Wishes program aims to guide important conversations about the end-of-life care you want to receive should you become seriously ill. This document can provide peace of mind for yourself and your loved ones in a stressful situation.
Featured in TV and in the pages of newspapers and magazines, the Five Wishes document has been called the first "living will with a heart and soul".
